Upload your documents

  • Photo ID

We need this to confirm who you are β€” a driver's license, state ID, or passport works.

  • Proof of Income

This helps us match you to free or low-cost services. A pay stub, benefits letter, or tax return works.

  • Proof of Address

Confirms you live in Washington State. A utility bill or lease with your name and address is perfect.

  • Medicaid CardOptional

Unlocks free rides to any medical appointment. A photo of the front of your Medicaid card is enough.

  • Medicare CardOptional

If you're on Medicare Advantage, your plan may include free rides. Front of the card is enough.

  • Disability DocumentationOptional

A doctor's note or disability award letter β€” only needed if a disability affects how you travel.

  • Insurance CardOptional

Some programs need a copy of your health insurance card. Front of the card is enough.

  • Veteran's ID or DD-214Optional

Unlocks FREE rides to VA appointments. A VA ID card or DD-214 discharge paper works.
