If you're on Medicare Advantage, your plan may include free rides. Front of the card is enough.
- Disability DocumentationOptional
A doctor's note or disability award letter β only needed if a disability affects how you travel.
- Insurance CardOptional
Some programs need a copy of your health insurance card. Front of the card is enough.
