Unlocks free rides to any medical appointment. A photo of the front of your Medicaid card is enough.
- Medicare CardOptional
If you're on Medicare Advantage, your plan may include free rides. Front of the card is enough.
- Disability DocumentationOptional
A doctor's note or disability award letter — only needed if a disability affects how you travel.
- Insurance CardOptional
Some programs need a copy of your health insurance card. Front of the card is enough.
- Veteran's ID or DD-214Optional
